Astellas Pharma
Japanese pharma giant behind prostate cancer drug Xtandi and a growing oncology lineup.
Astellas is growing today on Xtandi's peak sales, while betting that Padcev and its Strategic Brands can replace that revenue before US patent and price cuts bite in 2027.
Astellas makes and sells prescription medicines worldwide, generating billions in profit each year. The company's business is hard to copy because the government must approve each drug, factories must meet strict rules, and insurance companies have long-standing relationships with Astellas, plus patents protect existing drugs. However, its biggest moneymaker, a cancer drug called Xtandi, loses patent protection in 2027 and will get cheaper due to new U.S. rules, which will cut the company's income sharply within the next three to five years. The company is betting that newer drugs, especially one called Padcev, will grow fast enough to make up the difference, but nobody yet knows if that will actually happen.
Strategic Brands (Padcev, Izervay, Vyloy, Veozah, Xospata) combined YoY growth rate in quarterly filings through FY2027. Growth staying above ~30% shows new drugs are offsetting Xtandi's decline as planned.
FY2026-2027 Xtandi US sales figures versus the guided ~5.3% decline. A steeper drop than guided would show patent cliff and IRA pricing hitting harder than expected.
- FY2025 revenue ¥2,139.2B, up 11.9% YoY
- Xtandi global sales ¥960.8B in FY2025
- Strategic Brands revenue ¥480.3B, up 43% YoY
- Xtandi loses US exclusivity and faces IRA price cuts starting 2027, threatening ~45% of revenue.
- Strategic Brands must scale from ~23% to over 50% of revenue by 2030 to offset Xtandi decline.
- Pending generic-entry litigation (Ascent Federal Circuit appeal, May 2026) could accelerate Xtandi erosion.
- Ongoing facility closures (Seattle, South San Francisco) signal continued restructuring and headcount pressure.
| Rival | Direction | Evidence |
|---|---|---|
| Bayer/Orion (Nubeqa) | gaining on | Nubeqa taking prostate-cancer share, later patent expiry ~2038 vs Xtandi's 2027, noted 2026-04 |
| Johnson & Johnson (Erleada) | gaining on | Erleada directly rivals Xtandi in prostate cancer, J&J oncology sales grew 17.8% Q1 2026 |
| Pfizer | contesting | Co-markets Xtandi/Padcev with Astellas yet faces its own 2026-2030 patent cliff |
| Merck (Keytruda) | contesting | Combination partner for Padcev but also incumbent oncology giant facing 2028 cliff |
| Generic makers (Lupin, Zydus, Ascent, Qilu) | contesting | ANDA litigation over Xtandi/Myrbetriq; Ascent Federal Circuit appeal pending May 2026 |
- Is Astellas Pharma profitable in 2026?
- Yes. FY2025 (ended March 31, 2026) core operating profit hit ¥382.6 billion, up 832.4% YoY, on revenue of ¥2,139.2 billion, up 11.9% YoY. FY2026 guidance projects core operating profit above ¥600 billion, +12% YoY.
- What happens to Astellas when the Xtandi patent expires?
- Xtandi, roughly 45% of Astellas revenue at ¥960.8 billion in FY2025, loses US exclusivity and faces IRA price cuts starting 2027 (Europe exclusivity ends 2026). FY2026 guidance already forecasts Xtandi sales declining about 5.3%.
- Could AI replace Astellas Pharma?
- No. Astellas uses AI (Dyno Therapeutics, FRONTEO, Evinova) to speed drug discovery and trial design, but approval authority, decades of clinical trial data, manufacturing infrastructure, and payer relationships cannot be replicated by AI, as of April 2026 partnership announcements.
- Who is Astellas Pharma's biggest competitor?
- In prostate cancer, Bayer/Orion's Nubeqa and Johnson & Johnson's Erleada are actively taking share from Xtandi, with Nubeqa benefiting from a later patent expiry around 2038 versus Xtandi's 2027, per market analysis cited April 2026.
- Is Astellas Pharma laying off employees?
- Astellas closed its Seattle stem-cell (Universal Cells) site in April 2026, affecting about 55 employees with final closure by 2028, following a South San Francisco gene-therapy plant closure completed March 2025 that affected about 100 employees.
- Who owns Astellas Pharma?
- Astellas Pharma is a publicly traded company on the Tokyo Stock Exchange (4503), with no controlling parent. It was formed April 1, 2005 through the merger of Yamanouchi Pharmaceutical and Fujisawa Pharmaceutical, marking its 20th anniversary in April 2025.
