The call

Cigna's scale and licensing keep it locked in as a co-leader alongside UnitedHealth, with margin pressure from PBM pricing reform as the main thing to watch.

Cigna sells health insurance and runs Express Scripts, the largest pharmacy benefit manager in the U.S., handling prescriptions for nearly 200 million people. It is hard to replace because it has state licenses, decades of medical claims data, and relationships with employers and brokers that new competitors cannot easily copy, and AI alone cannot match its scale or regulatory approval. The biggest change ahead is new rules from the FTC that will force Cigna to stop hiding the real cost of drugs behind rebates and instead show prices upfront by 2027 and 2028, which will shrink profits on a large part of its business.

Why
01
AI pressure: None apparent. Cigna runs its own AI push, Pharmacy Forward, to cut internal costs rather than facing an AI rival eating its book. Could AI do the core job? Unlikely.
02
Bigger players: UnitedHealth Group (Optum Rx / UnitedHealthcare). Optum Rx competes directly for the same PBM and employer contracts, but both face the same FTC pricing reforms. Could a giant copy it? No, they are blocked.
03
What protects it. State insurance licenses, PBM regulatory footprint, employer distribution, and claims data across 190M lives block new entrants.
What would change this call
FTC settlement outcome for OptumRx (UnitedHealth) on PBM spread/rebate pricing. Would show whether industry-wide PBM pricing reform resets Evernorth's margin economics.
Express Scripts share of fully insured book moving to cost-plus pricing by 2027. Would show how much Cigna's core PBM profit model is being forced to change.
Cigna's Q2/Q3 2026 medical membership trend excluding the HCSC divestiture. Would clarify whether membership losses are one-time or reflect ongoing customer attrition.

Questions people ask
Is Cigna Group profitable in 2025?
Yes. Cigna Group reported FY2025 total revenues of $274.9 billion, up 11% year over year, with shareholders' net income of $6.0 billion ($22.18 per share) and adjusted income from operations of $8.0 billion, as reported February 5, 2026.
Is Cigna doing layoffs?
Yes. In February 2026 Cigna confirmed cutting about 2,000 jobs, under 3% of its workforce, as a cost-efficiency measure completed by end of February 2026, alongside otherwise strong Q4 2025 results and additional smaller New Jersey and Arizona cuts.
Who is Cigna Group's biggest competitor?
UnitedHealth Group, via Optum Rx and UnitedHealthcare, is Cigna's closest co-incumbent rival across both insurance and pharmacy benefit management. As of June 2026, the 'Big Three' PBMs, Express Scripts (Cigna), CVS Caremark, and OptumRx, process about 80% of U.S. prescription claims.
Could AI replace Cigna's business?
No. Cigna's moat rests on insurance/PBM licensure, employer distribution, and proprietary claims data across 190 million customer relationships, barriers AI cannot replicate. Instead Cigna deploys AI internally, launching the $100 million Pharmacy Forward program via Evernorth on July 1, 2026.
Who owns Cigna Group?
Cigna Group is a publicly traded, widely held corporation on the NYSE under ticker CI, with no controlling shareholder. Its market cap was approximately $74.45 billion as of July 20, 2026, with a 52-week share price range of $239.51 to $315.47.
Who is the CEO of Cigna Group?
Brian Evanko became CEO effective July 1, 2026, succeeding David Cordani, who had led Cigna since 2009 and moved into the Executive Chair role. Evanko previously served as President and COO and has been with the company since 1998.
