The call

Healius sits behind physical and regulatory barriers that keep competitors and AI substitution out, but its thin margins depend on a turnaround Sonic Healthcare doesn't need to attempt.

Healius runs about 2,158 medical testing collection centers across Australia, processing blood tests and similar samples. It has real advantages that are hard to copy: government accreditation, a physical network spread nationwide, relationships with thousands of doctors, and integration with Medicare billing. But it makes almost no profit (only 1.3 cents profit per dollar of sales), squeezed by what Medicare pays and rising worker costs. Its much bigger rival Sonic Healthcare controls about 40% of the market versus Healius's 26%, and Sonic makes far better profits on similar work. The company is gambling on jumping to much better profit margins by mid-2027, which would require volume to stay flat or shrink while costs drop sharply. Watch the full-year results in August 2026 to see if profit margins are actually climbing or if the turnaround is stalling.

Why
01
AI pressure: AI-based digital pathology and image analysis tools (e.g., PathAI-style slide reading). Automated slide analysis could cut pathologist hours per test over time, but hasn't touched Healius's volumes yet. Could AI do the core job? Unlikely.
02
Bigger players: Sonic Healthcare (ASX:SHL). Sonic runs a bigger pathology network with about 40% share versus Healius's 26%, competing for the same referrers and Medicare volume. Could a giant copy it? No, they are blocked.
03
What protects it. NATA accreditation, Medicare billing integration, and a 2,158-site collection network built over decades protect it from new entrants and remote substitution.
What would change this call
FY26 full-year results around 9 August 2026 show underlying EBIT margin still near 1-2%. Would show the turnaround is stalling, not closing the gap to Sonic's profitability.
Pathology volume growth turns negative again in H2 FY26 reporting. Would confirm patients or referrers shifting elsewhere, likely toward Sonic or Australian Clinical Labs.
Agilex Biolabs sale completes at a price materially below the roughly A$200m estimate. Would signal weaker asset quality or balance sheet pressure forcing a discounted exit.
Key numbers
  • FY25 revenue A$1,344.2m, up 5.7% YoY (2025-06-30)
  • FY25 underlying EBIT only A$17.1m, 1.3% margin (2025-06-30)
  • Pathology volumes down 0.4% over 10 months to April 2026 (2026-04-30)
Named risks
  • Underlying EBIT margin of just 1.3% in FY25 leaves little buffer against rising labour costs. Severity: high
  • Medicare rebate constraints cap pricing while pathology volumes have been flat to declining through 2026. Severity: high
  • Turnaround plan requires a 5 to 6x margin leap to high-single-digit EBIT by June 2027, an aggressive target. Severity: medium
  • Ongoing sale processes (Agilex Biolabs) and prior takeover approaches create ownership and strategic uncertainty. Severity: medium
  • Rising consumer AI self-diagnosis (59% of Australians in 2025) could shift demand patterns for basic health information. Severity: low

Questions people ask
Is Healius profitable in 2025?
No. Healius reported a net loss after tax of A$151.2 million for FY2025 (year ended 30 June 2025), though this improved from a A$645.8 million loss in FY2024. Underlying EBIT was just A$17.1 million on A$1,344.2 million revenue, a 1.3% margin, per its FY25 ASX results announcement dated 21 August 2025.
Who is Healius's biggest competitor?
Sonic Healthcare (ASX:SHL) is Healius's dominant rival, holding about 40% of pathology labs in private hospitals nationally versus Healius's roughly 26%, per the ACCC's Public Competition Assessment dated 16 December 2025. Sonic is larger, more profitable, and globally diversified across the US and Europe.
Could AI replace Healius's pathology labs?
Not easily. Core diagnosis requires liability-bearing pathologist sign-off, NATA accreditation, physical specimen collection across roughly 2,158 centres, and Medicare billing integration. As of August 2025 Healius had only three AI use cases in design, targeted to go live 1 January 2026, mostly augmenting workflow, not replacing lab functions.
Is Healius a takeover target in 2026?
Healius has fielded multiple approaches: Australian Clinical Labs' hostile bid was blocked by the ACCC in December 2023, Partners Group's A$2.12 billion proposal was rejected, and in May 2026 Healius engaged UBS to explore selling its Agilex Biolabs unit after unsolicited approaches from parties including Blackstone and IQVIA.
Is Healius cutting jobs?
Yes, headcount fell to 8,224 total employees as of 30 June 2025, down 1,434 or 14.85% year over year, per stockanalysis.com's aggregation of Healius's own filings. This partly reflects the May 2025 sale of Lumus Imaging and an active cost-reduction program targeting A$15 to 20 million in annualised savings.
Does Healius pay a dividend?
Healius paid a special dividend of 41.3 cents per share in May 2025 funded by Lumus Imaging sale proceeds, but the Board skipped the FY2025 final dividend entirely, stating intent to resume dividends when practicable, per the FY25 results announcement dated 21 August 2025.
