The call

The AI and hospital relationships look solid; the risk sitting next to them is a leveraged balance sheet and two active insurer lawsuits over billing.

Radiology Partners is a company that employs radiologists and uses AI software (called Mosaic) to help them read medical images faster and more accurately. The AI is genuinely hard for competitors to copy because Radiology Partners has trained it on 55 million images and got FDA approval for it, which gives them real advantages over rivals like RadNet. However, the bigger problem is not the AI itself, but how Radiology Partners bills insurance companies: it is being sued by Aetna and UnitedHealthcare for billing practices, is losing most of its surprise billing disputes, and could face new rules from the government that would shrink its revenue. The company also borrowed a lot of money to buy up other radiology practices, so if the billing model breaks down, it could struggle to pay back that debt, similar to what happened with another healthcare company called Envision.

Why
01
AI pressure: None apparent. AI feeds this business: Cognita/Mosaic speeds up its own radiologists, it doesn't replace the signature they legally need. Could AI do the core job? Unlikely.
02
Bigger players: RadNet (DeepHealth AI, outpatient imaging centers). RadNet already sells its own AI reading tools to hospitals, competing for the same health-system imaging contracts. Could a giant copy it? Probably.
03
What protects it. A licensed radiologist has to sign every read, plus contracts with 17 of the 20 largest US health systems and a huge proprietary scan archive feeding its AI.
What would change this call
11th Circuit rules on Aetna v. Radiology Partners appeal. Shows whether RP's out-of-network billing model survives legal challenge.
CMS issues new rules tightening No Surprises Act arbitration. Would directly cut into RP's arbitration-heavy revenue engine.
Moody's issues another credit downgrade on Radiology Partners. Would confirm deleveraging concerns despite 2025 debt refinancing.
Key numbers
  • Reads 53-55M+ imaging exams per year (2023-04-19)
  • Refinanced $2.3B first-lien debt, maturity pushed to 2032 (2025-07-01)
  • Moody's flagged ~10x debt-to-EBITDA at one point (2023-04-20)
Named risks
  • Aetna 11th Circuit appeal could reverse dismissal and reopen billing fraud liability. Severity: high
  • CMS could tighten No Surprises Act arbitration rules given RP's 28% share of national IDR claims. Severity: high
  • High leverage near 2.3 to 2.7 billion dollars in debt creates Envision Healthcare style bankruptcy risk if reimbursement shifts. Severity: medium
  • UnitedHealthcare lawsuit could expand into broader payer disputes affecting revenue collection. Severity: medium
  • AI productivity gains from Mosaic could eventually reduce need for radiologist headcount growth. Severity: low

Questions people ask
Is Radiology Partners profitable in 2026?
Radiology Partners reported over $3 billion in annual revenue as of its December 2024 Aetna lawsuit complaint, but it also carries roughly $2.3 to $2.7 billion in debt refinanced in mid 2025, and Moody's previously downgraded its debt citing leverage near 10x debt to EBITDA, making net profitability unclear.
Could AI replace radiologists at Radiology Partners?
No. RP's Mosaic subsidiary deployed Cognita based AI tools like Mosaic Reporting in June 2026 to speed up reads by up to 76 percent, but every report still requires a licensed radiologist's signature, so AI augments RP's 4,000-plus radiologists rather than replacing them as of 2026.
Is Radiology Partners facing legal trouble?
Yes. Aetna sued RP for alleged billing fraud; a Florida court dismissed the case April 16, 2026, but Aetna appealed to the 11th Circuit on May 6, 2026. UnitedHealthcare filed a separate suit in August 2025 that remained active with pending motions through July 2026.
Who is Radiology Partners' biggest competitor?
RadNet is the closest adjacent rival, operating 440-plus outpatient imaging centers and its own DeepHealth AI division, though RadNet owns imaging centers while RP staffs radiology practices at hospitals, making them competitors for AI leadership and health system contracts rather than direct substitutes as of mid 2026.
Does Radiology Partners own its own AI technology?
Yes. RP's subsidiary Mosaic Clinical Technologies acquired Stanford founded Cognita Imaging for $80 million in November 2025, and Cognita CXR received FDA Breakthrough Device Designation in March 2026, described as the first radiology generative AI model to receive it.
How much debt does Radiology Partners carry?
Radiology Partners refinanced approximately $2.3 billion in first lien debt in July 2025, extending maturities to 2032, and separately arranged $2.715 billion in debt financings including $900 million in notes and $1.815 billion in credit facilities as of mid 2026.
