The call

Somatus has scale and 100% retention in kidney value based care, but an unresolved CEO situation and an investor who could become a rival keep this unsettled.

Somatus manages care for about 500,000 kidney patients and makes money by preventing them from needing dialysis, the opposite of how traditional dialysis companies like DaVita and Fresenius make their profits. This creates a real advantage because the old dialysis companies earn more when patients stay sick longer, so they have less reason to prevent kidney failure. However, the company faces three big problems: no one knows for certain it can make money reliably (another company called Cano Health tried similar value-based care and stumbled), it has growing competitors like Strive and Monogram chasing the same patients, and its leadership is in transition. The one thing that could flip the picture overnight is UnitedHealth/Optum, which invests in Somatus but also competes in kidney care, if Optum decides to build or buy its own kidney-care program aggressively, Somatus's room to grow shrinks fast.

Why
01
AI pressure: None apparent. No AI product is running kidney risk prediction or care coordination in a way that has touched Somatus yet. Could AI do the core job? Unlikely.
02
Bigger players: UnitedHealth/Optum. Optum Ventures is both an investor and a vertically integrated care operator, so it could build or buy a competing kidney program instead of backing Somatus. Could a giant copy it? Probably.
03
What protects it. Deep clinical workflows and provider relationships built over 500,000 patients across all 50 states are hard to copy fast.
What would change this call
Somatus names a permanent CEO or announces a sale/acquisition within 12 months. Resolves whether the interim-CEO period reflects planned succession or distress.
Optum Ventures or UnitedHealth launches or acquires a competing kidney value-based care unit. Shows Somatus's investor turning into a direct competitor, compressing its market room.
CMS announces changes to ESRD Treatment Choices or Kidney Care Choices payment models. Would materially affect the revenue basis Somatus's contracts depend on.

Questions people ask
Is Somatus profitable in 2026?
Somatus discloses no audited revenue or profit figures. It claims 100% customer and provider partner retention in 2024 and YTD 2025 (July 22, 2025 press release) and manages over $14B in healthcare costs, but profitability itself remains unverified by any independent source.
Who is Somatus's biggest competitor?
Somatus competes most directly with Strive Health, InterWell Health, and Monogram Health in value-based kidney care, while dialysis incumbents DaVita Kidney Care and Fresenius Health Partners run their own competing CMS value-based kidney programs as of 2026.
Could AI replace Somatus's business?
AI could automate Somatus's RenalIQ risk-stratification analytics, which the company itself now brands 'AI-driven' (July 2025). But its in-home clinical care teams and multi-year payer risk contracts, built over roughly nine years, are not replaceable by software alone.
Who owns Somatus?
Somatus is privately held and venture-backed, with investors including Wellington Management, Optum Ventures, Deerfield Management, Elevance Health, Inova Health System, GIC, and Fidelity, per a February 28, 2022 Coverager report on its $325M Series E round.
Is Somatus a unicorn?
Yes. Somatus was valued above $2.5B at its Series E round closed February 23, 2022, according to FierceHealthcare, matching its listed unicorn status; a later unconfirmed secondary-market data point suggested a ~$4.98B valuation in June 2025.
Has Somatus had layoffs?
No layoff announcement has been publicly confirmed, but Revelio Labs workforce data show Somatus headcount fell from a 2023 peak of 1,051 to about 959 employees by March 2026, even as the company reported over 1,200 employees in November 2024.
