The call

A small team chasing a real gap between AI labs and clinical data, racing a well-funded data consortium and the labs' own pipelines.

BioStack sells patient records and clinical trial data to artificial intelligence companies that want to train models on real healthcare information. Right now, nobody else is doing exactly this, but Truveta (which has 120 million patient records and money from big pharmaceutical companies) and OpenAI (which bought a health company in early 2026) are both entering the same space. The hardest thing BioStack has to prove in the next year is that hospitals and AI labs will sign long-term exclusive deals with them instead of working with bigger competitors or building their own data pipelines from scratch.

Why
01
AI pressure: OpenAI (Torch Health, ChatGPT Health) and other frontier labs building in-house data pipelines. AI labs may build their own clinical reward-function pipelines instead of buying from a small vendor like this. Could AI do the core job? Yes, likely.
02
Bigger players: Truveta. Truveta already has 120M+ patient records, a 30+ hospital consortium, and a shipped EHR LLM that could extend into reward-function data. Could a giant copy it? Yes, easily.
03
What protects it. Founders' direct research relationships with AI labs and scientists, not yet backed by any exclusive data contract.
What would change this call
BioStack announces a named, exclusive data deal with a hospital, CRO, or biobank by Q2 2027. Would show BioStack's data moat is real and defensible against Truveta.
BioStack converts any of its 17 prospects into a named, paying AI-lab contract. Would show frontier labs choose to buy rather than build in-house.
OpenAI or Microsoft ships a healthcare RL-reward or benchmark tool via Torch Health or Copilot Health. Would show frontier labs are in-sourcing the niche BioStack targets.
Key numbers
  • 18 employees, based in San Francisco (2026-07-13)
  • 17 customers and prospects claimed, including AI labs (2026-05-25)
  • Hiring RL engineer at $200K-$250K plus 0.5-1.0% equity (2026-05-01)
Named risks
  • Proprietary data moat unverified at scale versus Truveta's 120M-patient consortium. Severity: high
  • Frontier AI labs insourcing data pipelines via acquisitions like Torch Health reduce BioStack's customer base. Severity: high
  • Pre-revenue with only $500K raised limits runway against well-funded rivals. Severity: medium
  • Handling clinical data carries HIPAA and consent-related regulatory exposure. Severity: medium
  • Small 18-person team competing against Big Tech and $320M-funded incumbents. Severity: medium

Questions people ask
Is BioStack Platforms profitable in 2026?
No. CB Insights data dated 2026-03-31 lists BioStack Platforms' revenue as $0, consistent with a pre-revenue startup that has raised only $500,000 total as of June 2026 and employs about 18 people as of July 2026.
Who owns BioStack Platforms?
BioStack Platforms is privately held, founded around October 2025 by CEO Sanat Mishra (cancer genomics background) and a co-founder/CTO ex-AWS/MIT. Backers include Y Combinator, Afore Capital, Heroic Ventures, Verdict Capital and Lobster Capital, per a June 2026 Caplight listing.
Could AI replace BioStack Platforms?
Not directly, since BioStack builds AI infrastructure itself, but its customers (frontier AI labs) are insourcing similar pipelines: OpenAI acquired Torch Health for about $100M in January 2026 and launched ChatGPT Health the same month, cutting demand for outside vendors.
Who is BioStack Platforms' biggest competitor?
Truveta is the closest scaled rival: it raised a $320M Series C in 2025, aggregates 120M+ US patient records across a 30+ health-system consortium, and already serves Pfizer, Moderna, Boston Scientific and Boehringer Ingelheim, dwarfing BioStack's $500K raised and 18 staff.
How many customers does BioStack Platforms have?
BioStack's own May 2026 YC launch post claims it grew from one customer conversation into 17 customers and prospects, including unnamed top AI labs, but this is a founder-reported figure with no independent confirmation or named contracts.
What does BioStack Platforms actually build?
BioStack structures fragmented clinical and preclinical biomedical data (EHR notes, labs, imaging, assays) into ML-ready training environments, evaluation benchmarks, and reinforcement-learning reward functions, sold to AI labs and biopharma clients, per its YC company page dated 2026.
