Fresenius Medical Care
World's largest dialysis provider, running clinics and making the machines patients depend on.
Two companies run most of America's dialysis chairs, and no software or algorithm changes who owns the machines and the licenses.
Fresenius Medical Care operates dialysis clinics and makes dialysis machines, treating patients with kidney failure three times per week. It is hard to replace because dialysis requires licensed staff, expensive equipment, and long-term contracts with insurance companies and hospitals, plus the company owns its own equipment maker. DaVita is its only real competitor (they split the market 77%), and AI cannot do the physical work of connecting needles or running machines. The thing that could change everything is if antitrust lawsuits force the company to lower prices to commercial insurance plans, which would hurt profits but probably not put the company out of business.
FTC decision on physician non-compete clause probe reported in 2024. Would show whether regulators restrict a tool FME uses to lock in referring physicians.
Execution outcome of the 5008X CAREsystem US rollout planned for 2026. Would show whether FME's equipment platform strengthens its integration moat versus DaVita.

- Is Fresenius Medical Care profitable in 2026?
- Yes. FME's operating margin hit 11.3% in FY2025, up from 7.9% in 2022, with operating income excluding special items growing 27% in 2025. Guidance for 2026 is broadly flat revenue but margin held within its mid-term target band, per its February 24, 2026 earnings release.
- Could AI replace Fresenius Medical Care?
- No. Dialysis requires physical machines, needles, and licensed clinical staff in-person; AI only augments decision-support and back-office layers. FME is building AI tools via a January 2026 SAP partnership and its kinexus platform previewed May 27, 2026, but core treatment delivery stays human and physical.
- Who is Fresenius Medical Care's biggest competitor?
- DaVita Inc. is FME's primary rival; together they held about 77.1% of US dialysis facilities in 2019, up from 59.1% in 2005, forming an entrenched duopoly. As of a November 14, 2025 estimate, FME held about 38% US share versus DaVita's 35%.
- Who owns Fresenius Medical Care?
- Fresenius Medical Care is publicly traded on Frankfurt (FME) and NYSE (FMS). Parent Fresenius SE remains its largest shareholder at approximately 29% of share capital as of December 31, 2025, following a November 2023 legal deconsolidation from an AG structure.
- Is Fresenius Medical Care laying off employees?
- Headcount has declined from about 112,000 at end of 2024 to roughly 110,000 by December 31, 2025, driven by divestitures of 391 facilities and about 12,600 employees since 2023 as part of an ongoing portfolio optimization program.
- Does Fresenius Medical Care face antitrust risk?
- Yes. A September 2025 amended class-action complaint in Colorado federal court alleges FME and DaVita conspired to fix prices and allocate markets, and the FTC reportedly opened a 2024 probe into physician non-compete clauses. Rulings expected in the 2026 to 2027 window could cap commercial pricing power.
