The call

Judi Health built its business on being the transparent alternative to the Big 3, and now federal rules are pushing the whole industry toward that same transparency.

Judi Health processes prescription claims and drug pricing for health insurance companies and employers. Big competitors like CVS, Cigna, and UnitedHealth have been hiding their real costs from patients and doctors through complicated pricing tricks, but new federal rules starting in 2026 will force everyone to be transparent, which erases Judi's main selling point of being 'the honest one.' The only way Judi's investors make real money is if their software-licensing business to other insurers grows fast enough before their pricing advantage disappears.

Why
01
Bigger players: CVS Caremark, Cigna Express Scripts, UnitedHealth OptumRx. FTC settlements are forcing the Big 3 to adopt rebate transparency by 2026-2028, copying Judi's core pitch at much larger scale. Could a giant copy it? Probably.
02
What protects it. PBM licensure, fiduciary status, a 65,000-pharmacy network, and 50M+ contracted lives make the platform hard to switch away from or rebuild quickly.
What would change this call
Whether Judi Cloud SaaS licensing revenue growth is reported separately in 2026-2027 filings or press. Shows if platform licensing is reaching software-like margins as promised.
Per-life or per-claim fee levels Judi charges after the 2028 Medicare Part D delinking mandate takes effect. Shows whether Judi's flat-fee pricing compresses like industry peers under new regulation.
Whether Caremark, Express Scripts, or OptumRx announce a delinked, transparent-pricing product post-FTC settlement. Shows if a Big 3 incumbent neutralizes Judi's core transparency pitch.

  • 2028 Medicare Part D delinking and CAA rebate pass-through mandate could commoditize Judi's core pricing differentiator. Severity: high
  • Crowded field of well-funded transparent-PBM rivals with identical positioning squeezes differentiation. Severity: medium
  • Judi Cloud platform licensing must scale to SaaS-like margins before flat-fee take rates compress industry-wide. Severity: medium
  • Publicly reported revenue and headcount figures are inconsistent across sources, limiting verifiable scale. Severity: low

Questions people ask
Is Judi Health profitable in 2026?
Judi Health has not disclosed profit figures. Forbes reported on September 23, 2025 that its PBM revenue was projected to hit $3.7 billion in 2025, up 75% from $2.1 billion, but that is pass-through claims revenue, not profit margin, so profitability remains undisclosed.
Who is Judi Health's biggest competitor?
The Big 3 PBMs, CVS's Caremark, Cigna's Express Scripts, and UnitedHealth's OptumRx, control about 80% of U.S. prescriptions as of July 2026 per Goodwin Law. Transparent-pricing rivals like SmithRx and AffirmedRx also compete directly for the same employer contracts.
Who owns Judi Health?
Judi Health is privately held and venture-backed. Its Series F round closed September 23, 2025 at $400 million, including $252.89 million in equity, led by Wellington Management and General Catalyst, valuing the company at $3.25 billion, up from $1.5 billion in March 2024.
Is it safe to take a job at Judi Health?
Judi Health said in a December 18, 2025 release that headcount had tripled since the start of 2024, and a September 25, 2025 report noted plans for about 250 new Charlotte jobs, indicating active hiring rather than layoffs, though exact current staff totals are unconfirmed.
Will new PBM regulation hurt Judi Health?
New rules including the 2026 Consolidated Appropriations Act mandate for 100% rebate pass-through and a 2028 Medicare Part D fee delinking, reported July 20, 2026 by Goodwin Law, push the whole PBM industry toward the transparent pricing model Judi Health already sells, risking commoditization of its main differentiator.
Has Judi Health raised money recently?
Yes. On September 23, 2025 Judi Health announced a $400 million round, including $252.89 million in equity, led by Wellington Management and General Catalyst, lifting its valuation to $3.25 billion.
