The call

GRAIL has the data and the lab but not yet the FDA green light, and after Galleri's trial missed its primary endpoint, Exact Sciences and Guardant are racing to get there first.

Grail makes a blood test that looks for early signs of cancer by studying DNA patterns. The company has real advantages: a large database of 174,000+ people, its own lab that meets strict quality rules, and a path through FDA approval that competitors would struggle to copy. However, rivals like Exact Sciences and Guardant Health are spending heavily to build their own similar tests, and there is no rule stopping them from trying. In February 2026, Grail's main test called Galleri failed an important trial run by the British health system, which means the FDA might not approve it for widespread insurance coverage. The one thing that matters most right now: whether the FDA approves Galleri for regular use, which would let insurance pay for it instead of patients paying $949 out of pocket.

Why
01
AI pressure: None apparent. No AI lab is close to shipping a clinical cancer-detection product; academic DNN methylation classifiers copy GRAIL's approach but lack its lab, data, or regulatory path. Could AI do the core job? Only partly.
02
Bigger players: Exact Sciences (Cologuard, Thrive MCED effort) and Guardant Health (FDA-approved Shield). Both have approved single-cancer tests and cash flow today, and are racing toward multi-cancer detection while GRAIL waits on one unproven test. Could a giant copy it? Yes, easily.
03
What protects it. A 174,000-plus participant proprietary dataset, its own CLIA/CAP lab, and a regulatory pathway rivals can't just copy overnight.
What would change this call
FDA issues a PMA decision (approval, rejection, or delay) on Galleri. Determines whether GRAIL's moat converts into reimbursed, scaled business or stays niche.
Guardant Health or Exact Sciences announces its own FDA submission or approval for a multi-cancer test. Shows a rival reaching the regulatory finish line first, undercutting GRAIL's lead.
CMS or a major payer issues a reimbursement decision on Galleri. Shows whether the test moves beyond cash-pay ($949) toward mainstream adoption.

Questions people ask
Is Grail profitable in 2026?
No. Grail posted a Q4 2025 net loss of $99.2M (reported 2026-02-19) and a Q1 2026 loss continued despite Galleri revenue growing 37% YoY to $39.8M. The company says its October 2025 $435M raise gives it cash runway into 2030, but profitability is not near-term.
Did Grail's NHS-Galleri trial succeed?
No. On 2026-02-19 Grail announced the 142,000-participant NHS-Galleri trial failed its primary endpoint, showing no statistically significant reduction in Stage III/IV cancer diagnoses, though it reported a 16% increase in Stage I-II detection. The stock fell about 50% that day.
Who owns Grail now?
Grail is a public company (Nasdaq: GRAL). It was acquired by Illumina for $7.1B in a deal announced September 2020, then forced to divest by EU regulators; the spinoff completed June 24, 2024, with Illumina retaining about a 14.5% stake.
Who is Grail's biggest competitor?
Guardant Health and Exact Sciences are the closest rivals. Guardant's Shield test already has FDA approval for colorectal screening and it presented multi-cancer detection data in April 2025, while Exact Sciences reports over $2.8B in 2025 revenue and is building its own MCED test after acquiring Thrive Earlier Detection.
Could AI replace what Grail does?
Not easily. Galleri's own product is a machine-learning methylation classifier, and academic groups have replicated similar classifiers, but Grail's proprietary dataset of over 174,000 trial participants plus its CLIA/CAP lab and pending FDA PMA filing (submitted January 2026) are not things a pure AI model can bypass.
Will the FDA approve Grail's Galleri test?
Unclear as of mid-2026. Grail completed its Premarket Approval submission to the FDA in January 2026, backed by PATHFINDER 2 and NHS-Galleri data, but no approval timeline has been disclosed, and the February 2026 trial endpoint miss weakened the case for a favorable decision.
