The call

The data and clinic relationships still work, but Roche keeps cutting pieces off the company while Tempus AI sells a rival version of the same idea.

Flatiron collects real-world medical information from cancer doctors across the United States, then packages that data for drug companies and hospitals to make better treatment decisions. The company built something genuinely hard to copy over 15 years: it has records on 6 million patient journeys, deep ties to 40% of community cancer clinics through their software, and FDA-grade credibility that took years to earn. But Roche, which owns Flatiron, started selling off pieces in late 2025, has cut 26% of staff since 2023, and is exploring further sales, which means the valuable data could scatter to different owners or competitors. A well-funded rival called Tempus AI already sits inside the same cancer clinic software and is building its own competing data platform. The biggest question mark: whether Roche finishes breaking Flatiron apart in the next year, and whether that breakup turns the data asset into something weaker and easier for Tempus or others to displace.

Why
01
AI pressure: Tempus AI. Tempus is public, well-funded, and already plugged into OncoEMR, selling its own precision-medicine data platform alongside Flatiron's. Could AI do the core job? Unlikely.
02
Bigger players: Roche (its own parent). Roche explored a full sale in 2024 and carved off the clinical trials unit to Paradigm Health in Dec 2025, shrinking the company from inside. Could a giant copy it? No, they are blocked.
03
What protects it. Fifteen years of abstractor-curated patient records and OncoEMR software running in 40% of US community cancer clinics.
What would change this call
Roche announces sale or further carve-out of Flatiron's remaining OncoEMR/RWD core within 12 months. would show Roche is exiting oncology data entirely, not just trimming clinical trials.
Flatiron headcount trend for 2026 (stabilizes vs. continues falling 10%+ year over year). shows whether Dec 2025 Paradigm carve-out was a one-time pruning or ongoing shrinkage.
Tempus AI announces a major pharma real-world-evidence contract or community-oncology EHR deal. would show Tempus actively winning business Flatiron previously supplied.
Key numbers
  • ~6 million patient journeys in database (Jun 2026)
  • OncoEMR covers ~40% of US community oncology sites (May 2026)
  • Headcount down 26% since 2023 (2026)
Named risks
  • Roche may divest remaining OncoEMR/RWD core after Aug 2024 sale exploration and Dec 2025 partial carve-out. Severity: high
  • Sustained headcount attrition, down 25.9% from 2023 to Dec 2025. Severity: medium
  • Tempus AI, well-funded and public, competes directly via overlapping OncoEMR integration. Severity: medium
  • Internal AI tool Telescope automates analyst work that once required Flatiron staff. Severity: low
  • New diagnostics integrations (Quest/AON) still in pilot phase, unproven at scale. Severity: low

Questions people ask
Is Flatiron Health profitable?
Flatiron Health discloses no standalone financials; its results are folded into Roche's pharmaceuticals segment. Roche paid $1.9B to fully acquire it in April 2018, and headcount has since fallen 25.9% from 2,033 in 2023 to 1,506 by December 2025, suggesting cost pressure rather than disclosed profitability.
Who owns Flatiron Health?
Roche has wholly owned Flatiron Health since completing a $1.9B acquisition on April 6, 2018 (total deal value about $2.1B including a prior 2016 stake). It operates as a legally separate 'independent affiliate' inside the Roche Group as of 2026.
Is Flatiron Health being sold or broken up?
In August 2024, the Financial Times reported Roche was exploring a full or partial sale via Citigroup. On December 4, 2025, Paradigm Health acquired Flatiron's Clinical Research Business, while Roche retained Flatiron's core OncoEMR and real-world-data operations.
Could AI replace Flatiron Health?
Flatiron argues no: its head of data science claims generic LLMs hit only about 60% accuracy on oncology cohort questions versus 90%+ for its own proprietary-trained models (company claim, May 2026). Flatiron instead ships its own AI layer, Flatiron Telescope, launched May 19, 2026.
Who is Flatiron Health's biggest competitor?
Tempus AI, a public precision-medicine data company, is the most direct live rival; it has been integrated into OncoEMR as a diagnostics-ordering partner since 2024. Oncology RWD specialists COTA Healthcare and Syapse also compete directly for pharma real-world-evidence contracts.
Is it safe to take a job at Flatiron Health right now?
Headcount fell from 2,033 employees in 2023 to 1,506 by December 2025, a 25.9% decline per Revelio Labs, and parent Roche explored selling the company in August 2024 before divesting its Clinical Research Business to Paradigm Health in December 2025.
